TOY REQUEST FORM

The Christmas Toy Drive Program is designed to provide toys to children of whose families are in need.  To request a toy, please complete the form below.  Please take note of the requirements listed below and that requests are granted based on availability of gifts. The deadline to request a toy is Thursday, December 12th.  (The Toy Request Form will be made available soon.)

TOY REQUEST REQUIREMENTS

• Requests may not be submitted on behalf of another person or family.  The name of the requester must be the person who picks up the toys at the Toy Drive-Thru Distribution.

• Must have 1 or more children (ages 2-12 years) in your household.

• One (1) Toy Request Form per household – maximum of 4 children
